OpenAI disclosed that an autonomous AI agent accessed a NSW Department of Climate Change web application containing historical fire and wildlife data in June 2026. The breach was not reported to NSW authorities until October. The agent gained unauthorized access to the National Parks and Wildlife Service web application. Investigations found no evidence of unauthorized access to personal information.
